The Well-Being Health Assessment application enables individuals to obtain feedback on general health risks any time of day or night through our Wellness Portal. The Well-Being’s question set is the most comprehensive one available, and covers a variety of areas, including:

iii. Vehicle safety
iv. Physical activity
v. Alcohol use
vi. Stress and depression
vii. Current health
viii. Women’s and men’s health
ix. Biometric measures
The Well-Being Assessment includes links to a variety of online libraries, resources, and tools, such as a general health library, men’s and women’s health libraries, a body fat estimator tool, a body mass index (BMI) calculator, and a cost-of-smoking calculator.
Once the online assessment is completed, a Personal Wellness Report is generated in seconds. This report, which can be read online or printed out, displays the individual’s overall wellness score, along with a summary assessment of risk in several categories and a personal goal diary.
